Implementing RPM in Community Health Centers

With the recent HRSA hypertension funding requirements to implement Bluetooth or wireless-enabled self-measured blood pressure devices, health centers are looking for guidance on how to roll out high-tech remote patient monitoring (RPM) tools. In response, this three-part RPM training series will kick off with a high-level overview of RPM, such as the difference between low tech and high-tech tools and how to choose an RPM tool. The second session will focus on best practices for RPM implementation and data collected. The final session for this series will be taking a deeper dive into the data and how to utilize it in a meaningful way.

This three-part webinar series will focus on the following:

Webinar 1 (High Level Overview)
1. What is Remote Patient Monitoring
2. Difference between low tech devices (self-reporting devices) & hi-tech devices (connected)
3. Best practices in selecting the appropriate devices for your patient population
4. Aligning RPM with organizational strategic initiatives
5. Understanding RPM program financial sustainability
6. Why data & trends are important in helping to improve patient outcomes

Webinar 2 (RPM from a clinical lens)
1. Importance of the care team in RPM- beyond the devices and data
2. RPM pre-implementation and post-implementation best practices
3. Education of the patient and setting up the program from a clinical lens
4. Alerts and how to best respond to them
5. Importance of a clinical champion
6. Patient engagement and clinical improvements

Webinar 3 (Data)
1. Patient data trends
2. Raw data vs. structured data
3. Movement of data into the EHR
4. Ownership of data
5. Case studies
6. Engaging with payers

By attending this course, participants will be able to:

Presenter Biography:

Huzefa Dossaji, PharmD, Telehealth & RPM Specialist

Huzefa has been involved in the health care IT & telehealth arena for 6 years. In this time, he has worked with FQHCs and PCA/HCCNs on designing and implementing HIT and Value Based projects particularly in the Telehealth, connected Remote Patient Monitoring, Virtual Scribe Models, and other areas in the HIT arena.

He was instrumental in working with the Louisiana Primary Care Association (LPCA) HCCN in initiating telehealth programs for their member FQHCs as well as a payer funded RPM program for patients with uncontrolled diabetes in collaboration with the North Carolina Community Health Care Association (NCCHCA). He is currently working with the South Carolina Primary Health Care Association (SCPHCA) and Hawaii Primary Care Association (HPCA) to help their member FQHCs implement telehealth and remote patient monitoring as part of their HRSA HCCN Grant Objectives. He has also worked with FQHCs in developing a long-term Telehealth strategy and implementation.

In the area of Cybersecurity, he collaborated in putting together SRA programs with NCCHCA, SCPHCA & Hawaii HCCN’s for their members.

He has presented at many PCA/HCCN & NACHC Conferences and has been invited to present at the 2021 NACHC FOM/IT Conference on Telehealth and RPM.

He is particularly passionate about helping and educating health centers with new initiatives around value-based care.

Huzefa has a background in both healthcare and education and teaches Kung Fu & Tai Chi in his spare time.
