Risk Stratification 101

Assessing risk among patient populations is a critical component of identifying patient needs and connecting them to beneficial resources. This training will provide (1) background and need for risk stratification across patient panels, (2) data commonly used to inform risk stratification criteria, (3) workflows and strategies to implement patient analysis and identification of risk, and (4) potential impacts on reimbursement. The session will include information and best practice sharing, breakout discussion groups, and sharing of actionable, publicly accessible resources to help adopt/adapt risk stratification in your practice setting. The course will focus mostly on adult medicine, but the principles are broadly applicable to other populations.

Participants in this training will:

  • Develop a broad understanding of risk stratification in the primary care setting
  • Gain access to actionable, publicly accessible materials to introduce risk stratification into practice
  • Understand practical workflow and policy considerations when implementing a risk stratification model
  • Tie the benefits and challenges of risk stratification to CalAIM Alternative Payment Mode (APM) and Enhanced Care Management (ECM) initiatives

Instructor Biography:

Dr. Bell is a results-oriented healthcare professional with extensive knowledge and application of medical home standards and best practices. She provides education and assistance to foster practice transformation to meet value-based payment program requirements, as well as guidance and assessment to assist with PCMH recognition. In her previous tenures, Dr. Bell worked for Amerigroup Community Care, NJ (Anthem), training practices in quality improvement, cost containment metrics and data analysis. She also served as the health plan liaison for the implementation of the Comprehensive Primary Care Initiative (CPCi) and the Comprehensive Primary Care Plus (CPC+) program in the New Jersey market. Dr. Bell received her Doctor of Medicine degree from Brown University and her Bachelor of Science in biology from Spelman College. Dr. Bell is a NCQA Patient Centered Medical Home Certified Content Expert and serves as a PCMH Evaluator for NCQA.
