Patient Adherence to Care Plan Goals and Effective Care Coordination Approaches After Care Plan Visit

FreeSuzzane Daub, LCSW of Health Management Associates will facilitate a 90-minute webinar focused on how to use motivational interviewing (MI) and trauma-informed care to increase patient engagement and adherence to care plan goals. This webinar will build upon the content shared in webinar 1 to provide enhanced care managers the skills needed to engage clients with addressing barriers to patient-centered goals. Attendees will participate in interactive engagement strategies such as “real-play” to facilitate a strong understanding of MI concepts.
At the end of this session, participants should be able to:
- Describe the spirit of motivational interviewing and how it aligns with the key principles of trauma-informed care.
- Utilize core MI skills (OARS) to elicit motivation for change and adhere to care plan goals.
- Learn strategies on how to engage clients in between care management visits.

Instructor Biography:
Suzanne Daub is a leading expert and nationally recognized trainer in integrated healthcare who knows how to help clients design, scale and evaluate behavioral integration into primary care and wellness culture. Suzanne has more than 30 years of experience in direct patient care, program administration and managed care. For 18 years, she served as director of behavioral health at a multisite Federally Qualified Health Center (FQHC) in Philadelphia where she integrated care using the Behavioral Health Consultation (BHC) model. Suzanne co-founded the largest network of primary care behavioral health providers in the country and was recognized by the Pennsylvania Association of Community Health Centers with the Innovations Award.
As the extreme health disparities among people with serious mental illness came to light, Suzanne brought her integrated care skills to community mental health organizations across the country as a senior integrated care consultant for the National Council for Behavioral Health.
