HCCN Best Practice Webinar – Data Stewardship

Data stewardship is the collection of practices that ensures a health center’s data is accessible, usable and trusted. Data stewardship not only refers to the tools utilized for tracking and reporting data, but it also refers to the people and the processes who are involved to ensure that EHR data is being accurately collected. After successfully implementing their own data stewardship program, the Comprehensive Community Health team will be presenting on their best practices for utilizing data to inform value-based care. You’ll learn everything from the tools the health center uses for data reporting/aggregation to the health center’s structure for relaying reports to outreach staff and care teams.

During this training, participants will learn:

  • How to use the data from your EHR to inform value-based care
  • How to validate that you are working with accurate data libraries
  • How to automate data reporting
  • The structure of an efficient data stewardship team
  • Data stewardship uses of i2iTracks

Instructor Biographies:

Jessica Clark, Practice Transformation Specialist, California Medical Association

Jessica Clark has thirteen years of experience as an LVN across several different areas of healthcare in California. Currently she is a Practice Transformation Specialist with California Medical Association’s Physician Services (CMA-PSO) where she provides technical assistance and process improvement support to several participating health care centers in the CCALAC HCCN, as well as supports a variety of other initiatives within CMA-PSO. Jessica has worked as a QI Data Analyst for a Northern California FQHC and has also been a HEDIS Coordinator and Practice Transformation Coach for a large California health plan. Before that, Jessica was an LVN Nurse Supervisor and Charge Nurse, and worked directly in patient care, specializing in wound care for Diabetic patients. Jessica is passionate about population health management and harnessing clean data to drive better outcomes for patients and health centers.

Raymond Retirado, VP of Quality and Clinical Programs, Comprehensive Community Health Centers

Raymond Retirado is a Registered Nurse and has eleven years of professional experience in the nonprofit healthcare industry. Bringing his experience as an LVN Charge Nurse from a skilled nursing facility, Raymond was employed by Comprehensive Community Healthcare Centers (CCHC) in October 2010. While working on the clinical floor, Raymond gained experience and understanding of CCHC’s clinical workflows, helping to further expand his clinical and operational knowledge. Upon obtaining his RN License, Raymond was provided the opportunity to function as CCHC’s Nursing Coordinator. In his more recent years, he’s had the opportunity to function as the Quality Improvement Director and is now serving as CCHC’s VP of Quality and Clinical Programs where he oversees, the CPSP, Title X and QI Programs.

Ani Keshishian, LVN, Quality Improvement Manager, Comprehensive Community Health Centers

Ani Keshishian is an LVN, with eleven years of nursing experience in the healthcare industry. Bringing her experience as a home health nurse, and GI and Hepatology nurse, Ani was employed by Comprehensive Community Health Centers (CCHC), in 2015 as an LVN Back Office Lead. In 2019, Ani transitioned to the role of Quality Improvement Coordinator. In her more recent years, she was promoted to the role of Lead Quality Improvement Coordinator and is now serving as CCHC’s Quality Improvement Manager. Her and her team’s goal is to support improved health care delivery and outcomes to those that CCHC serves and to promote continuous improvement within the organization.
