Cognitive Health Assessment Training: Assessing and Addressing Dementia in Primary Care

The Cognitive Health Assessment training empowers primary care providers and their teams to use a validated cognitive and functional screening tool, identify a patient’s support person, and develop a care plan. The Cognitive Health Assessment (CHA) is a 5 – 10-minute dementia screening approach intended for patients 65 and older. The assessment is validated in primary care, easy to score, and available in multiple languages.  

By the end of the training, attendees will be able to: 

  • Identify two validated cognitive and functional screening tools. 
  • Describe the frequency of conducting, scoring and interpreting the validated screening tools. 
  • Discuss the three components of a brain health plan for use with patients after conducting a cognitive health assessment. 
  • Describe the importance of documenting a patient’s health care agent in their medical record as part of the cognitive health assessment.

Instructor Biography:

Eddy Moreno, MPH, Director of Clinical Outreach

Eddy Moreno is the Director of Clinical Outreach for Alzheimer’s Greater L.A. and will support the outreach of the Dementia Care Aware initiative to improve the ability of primary care teams to detect dementia and create stage-appropriate care plans based on the needs of each individual. Eddy also served as a Community Development Consultant for Aetna Better Health of California supporting the strategic growth objectives of Aetna Medicaid. In this role, Eddy was responsible for establishing, nurturing, and maintaining relationships with community-based organizations, member and provider associations, and advocy organizations in Los Angeles, Riverside, and San Bernardino Counties.  

Maryam Rahimi, MD, University California, Irvine (UCI).

Dr. Maryam Rahimi, MD is a board-certified University California, Irvine (UCI) Health internal medicine physician who specializes in all aspects of primary care. Her clinical interests include preventive care, management of chronic illness, women’s health, cancer screening and advanced care planning. Rahimi earned her medical degree at Indiana University School of Medicine in Indianapolis, where she completed an internship and a residency in internal medicine. Her research interests include screening for dementia, hypertension in primary care and advanced care planning. She is a lead UCI Health investigator on a five-year study to develop better approaches to advance care planning for seriously ill patients in collaboration with UC San Francisco Health and UCLA Health. She sees patients at UCI Medical Center in Orange.
