LA County Street Medicine Coordination Collaborative
Convened by CCALAC, the collaborative aims to coordinate street medicine services among providers in LA County to ensure linkages to specialty care, medical homes, housing, and other supportive services. The collaborative aims to develop a network of street medicine providers that are aligned in the mission to provide consistent, comprehensive, patient-centered and trauma-informed care that addresses both the health and social needs of patients experiencing homelessness.
