
Empanelment Training Series: Fundamentals of Empanelment

FreeDr. Greg Vachon MD, MPH, Deena Pourshaban, MPH, and Leslie Brooks of Health Management Associates will co-facilitate a 60-minute webinar that will provide an overview of the fundamentals of empanelment, describe the methodology of assigning patients to provider panels, outline necessary staff roles to sustain empanelment processes and how to self-assess your own empanelment environment. Participants will have access to empanelment templates and tools. After the webinar, the facilitators will offer 30 minutes of office hours. Office hours will consist of addressing clinic-specific questions related to empanelment, such as how to improve continuity rates, reviewing health center data (in breakout rooms), and any other specific requests from health center members.
At the end of this session, participants should be able to:
- Understand the fundamentals of empanelment
- Know how to operationalize empanelment in their health center
- Have access to a self-assessment empanelment tool to understand their current empanelment environment

Instructor Biographies:
Dr. Greg Vachon has over 30 years of experience in clinical work and leadership in safety-net health care and has been a Principal at HMA for over 15 years, with a focus on ambulatory care, data analysis, and information technology to power success in population health and value-based care. Data analysis areas of focus include right-sizing and projecting workforce, assessing impact of population interventions, creating dynamic business plans in health care, and supporting implementation and analysis of various components of optimized primary care. A component of special interest over the past two decades has been empanelment. Greg has assisted nearly fifty FQHCs and other safety-net systems across the country implement effective empanelment procedures reflective of their environments since 2010.
Deena Pourshaban has over 12 years of healthcare and public health experience with extensive work in practice transformation, social determinants of health, quality improvement, Patient-Centered Medical Home implementation, health center operations, and collaborative work with clients and care teams to tackle difficult clinical quality measures. Deena has led many large-scale practice transformation initiatives, supported over 10 community health centers achieve PCMH recognition which includes implementing empanelment as one of the core requirements and recently co-lead the empanelment workstream supporting 32 community health centers implement participating under Kaiser Permanente Population Health Management Initiative implement empanelment processes and workflows.
Leslie Brooks has over 8 years of experience collaborating with health plans and healthcare teams to implement whole-person, person-centered approaches to healthcare integration. Her domains of expertise include population health management, data analytics, training and evaluation work. Ms. Brooks’ supported over 10 community health centers implement empanelment processes and workflows under Kaiser Permanente Population Health Management Initiative. Her empanelment work focused on developing, workflows, dashboards, policies and procedures and staff training.
