Los Angeles Practice Transformation Network (LA-PTN)
The Los Angeles Practice Transformation Network is an initiative to help clinicians achieve large-scale health transformation primarily in Diabetes and Depression care. The initiative is peer based learning networks designed to coach, mentor and assist clinicians in developing core competencies specific to practice transformation. Clinic networks strengthen the quality of patient care and spend their health care dollars more wisely by promoting broad payment and practice reforms in primary care, promoting care coordination and sustain learning and improvement across the health care system. This program is funded by L.A. Care Health Plan and is a sub-contract with CCALAC to provide project management and coaching to enrolled CCALAC Member Clinics.
