SUMMARY:Disaster Preparedness for People with Chronic Diseases in Medically Under-resourced Communities
DESCRIPTION:Wildfires\, storms\, floods\, and other disasters cause disruptions in medication access\, evacuations\, and power outages for millions of people in America each year. Many people either take medication or have an ongoing medical condition\, and survey findings suggest a significant portion of them would either have difficulty continuing their medications or finding support to help manage their illness during an emergency. When managed poorly\, chronic conditions\, such as diabetes\, kidney disease\, and heart disease\, can lead to medical crisis. Power is often interrupted during disasters\, affecting storage for temperature-sensitive medications and electricity-powered medical equipment. \nLed by Direct Relief staff\, this workshop provides an overview of the impact of disasters on medically under-resourced communities and people with chronic diseases. This workshop has a broad approach intended for clinicians and other health center staff concerned about disaster preparedness. She has also given lectures at NAFC symposiums on ways to supply medications to uninsured patients. Currently\, Ruth is the Senior Advisor at Direct Relief and previously served as the Director of Pharmacy and Clinical Affairs. Since 2006\, Ruth has expanded programming domestically including the development of toolkits for preparedness and response for nonprofit partners. Further\, she served on the state of Texas’ Blue Ribbon Task Force to better prepare for disasters. Prior to joining Direct Relief\, Ruth was the Pharmacy Director and first on staff pharmacist at Venice Family Clinic. \nAnnie Vu\, Associate Director of Charitable Pharmacy\, Direct Relief \nFor the last eight years\, Annie Vu has worked closely with community health centers\, free clinics\, and other safety-net facilities to provide ongoing support and resources to organizations impacted by emergencies. She has strong relationships and is a key partner that has served as a point of contact to partners by offering assistance\, expediting shipments\, resolving logistical challenges\, gathering stories and feedback\, and providing updates for internal/external communication. \nDirect Relief aims to provide support efficiently and effectively\, while recognizing that every community and event is unique. Projects led include two airlifts to Puerto Rico after Hurricane Maria\, extensive distribution of PPE and ICU kits during the pandemic\, and pre-positioning caches of medicines and medical supplies ahead of hurricane season each year. Most recently\, Annie has been supporting healthcare providers on Maui as they continue to recover and rebuild from the wildfires\, which includes shipments of medicines and supplies\, a vaccination campaign coordinated by the state department of health\, and PPE kits for safe re-entry. \n

She is responsible for End-User Training\, Tip Sheets\, Workflow Analysis\, Identifying Gaps\, and Optimal Configuration. During her time with Medical Advantage\, she has spent a great deal of time building out Workflow Solutions for eCW along with assisting the Implementation Team on Build Checklists for Go-Live Clinics. Tiffany also has vast knowledge with eCW EPCS\, Quality Programs\, Patient Engagement\, ASC\, Inventory\, In House Configuration\, eClniicalTouch\, eClinicalMobile\, Dictation solutions\, Letter and Form Management\, and P2P Workflows. \nTiffany has an extensive knowledge with healthcare processes working as Registered Nurse providing direct patient care in diverse settings both In-Patient and Out-Patient Care working with multiple EHR Software’s. Tiffany has worked in ICU\, Respiratory Step-Down Units\, Urgent Care Clinics\, and Primary Care Clinics using EPIC\, eClinicalWorks\, Meditech\, and Docutap. While employed with the Primary Care Clinic\, using eCW\, she was responsible for scrubbing all the providers notes for completion as the Assistant Director of Medical Services. She also has extensive knowledge with Information Technology\, building servers\, end user computers\, and networks prior to Nursing School as Network Administrator. She has actively worked as an EHR Consultant for the past 4 years honing her skills as a Nurse and a Network Administrator to provide solutions for complex challenges. She has worked alongside billing professionals identifying solutions for coding notes correctly for payment of services. She is an exceptional trainer providing thorough instructions identifying learning styles and adapting accordingly. Her skills as critical thinker\, collaborator\, and communicator assist her with providing solutions. \nTiffany obtained her Bachelor of Business Administration from Northeastern State University with a focus in Telecommunications and her Bachelor of Science in Nursing from University of Oklahoma. She also served on her State Nursing Student Association Board while attending nursing school as President and Public Relations Chair and is still involved in her State Nursing Association. And how can these complementary approaches work together to support internal organizational culture and\, ultimately\, patient care? It seemed that the focus was on symptoms and not the underlying cause of the problems. When she learned about the ACEs framework and trauma-informed approach\, a lightbulb went off. This was how she could support people through the process of resolving those pain points in helping support clients and their families heal. \nShe then shifted her focus and began training across sectors to amplify this approach to care. She draws from her personal experience and her professional education to encourage empowerment and equity. Andi has served a variety of populations including children with special needs\, at-hope youth\, adults with special vocational training needs\, and youth and families. Since moving to California in 2016\, she has served both youth in schools all over LA County as well as victims of the Malibu fires. She currently lives her mission through her work at Origins Training where she supports the integration of a trauma-informed approach with people and organizations and at Integrative Counseling where she provides 1:1 trauma-informed therapy through telehealth in Arizona. \nOutside of her work\, Andi spends her time doing yoga\, enjoying good food\, and mountaineering. \nLori Chelius\, MBA/MPH\, Co-Founder\, Origins Training & Consulting \nLori is driven by a desire to create sustainable change. \nPrior to founding Origins with Andi\, she spent 15 years pursuing this desire in a number of roles in healthcare and public health\, including strategic planning\, business development\, project management\, market research\, and needs assessment. During that time\, Lori saw opportunity after opportunity to address issues upstream and understand the root causes\, not just the symptoms. When Lori met Andi\, a fellow champion of a trauma-informed approach\, she realized they could team up to help organizations build strong foundations based on solid values\, build resilience\, and provide even better care for the people they serve. And so\, Origins was born. \nLori has a BA in psychology (with a concentration in neuroscience) from Williams College and an MBA and MPH from UC Berkeley. She lives in California with her wife\, three kids\, their dog (Benjie) and their two cats (Fred and George). She also coaches youth soccer (and still attempts to play it). \nGuest Speaker Biographies: \nDesirae Spignor\, LCSW\nDesirae is a Licensed Clinical Social Worker. Her therapeutic approach is eclectic\, integrating her education and lived experience into practice. She identifies trauma (historical\, environmental\, interpersonal\, etc.) as the root cause of most behavioral health and even some medical issues. Working on ways to alleviate the impacts of trauma(s) on overall wellness. With this in mind\, her work as a Behavioral Health Specialist focused on learning/unlearning about trauma and how it impacts us.\nDesirae’s greatest passion is creating safe spaces in which those impacted by systems and systemic oppressions can heal. She strives to empower individuals\, families and communities to find holistic wellness and strengthen relationships so they can be at peace\, feel whole and be safe. \nW. Suzanne Eidson-Ton\, MD\, MS\nDr. Eidson-Ton serves as Chief Medical Officer for CommuniCare+OLE. She brings more than 20 years of experience in medical leadership to the role. Prior to joining CommuniCare+OLE in Yolo County\, Suzanne served as faculty in the UC Davis Health Departments of Family and Community Medicine and OB/GYN. As faculty\, she served in various leadership roles\, including being director for the Rural PRIME program (a pipeline program for medical students from rural communities) and director of the UC Davis Family Medicine Residency Network. She continues to maintain a 20% faculty appointment at UC Davis Health\, where she provides inpatient maternity and newborn care and acts as liaison for many pipeline efforts with the School of Medicine\, School of Nursing and Department of Family and Community Medicine. She and her husband are empty nesters\, as their second child left for college this year. \nErika Roshanravan\, MD\nErika Roshanravan\, MD FAAFP\, is a family physician and has been working at CommuniCare+OLE for the past 5.5 years. She serves as Medical Director at the CommuniCare+OLE’s Davis Community Clinic site and as CommuniCare+OLE’s Lead Physician for Wellbeing and Trauma-Informed\, Resilience-Oriented Care. She completed medical school at the University of Bern in Switzerland and residency as well as an additional year as chief resident/ Acting Instructor at the University of Washington in Seattle WA. Committed to spending her career serving the most vulnerable\, she spent her first six years in practice at a community health center in Everett WA doing full-spectrum family medicine including obstetrics before moving to California and joining CommuniCare in 2018. Dr. Roshanravan completed the AAFP Leading Physician Wellbeing training program in 2022 and the Train New Trainers in Primary Care Psychiatry fellowship at UC Davis/UC Irvine in 2019. Her passion is in advocacy to address “all layers of the problem onion” (from the clinic site to the organizational level and communities\, to the healthcare system and society at large) in order to build a more equitable\, trauma-informed\, wellbeing-inducing healthcare system for all. As such\, in addition to her roles within her workplace\, she serves on the board of directors for the California Academy of Family Physicians (CAFP) and the Sacramento Valley Chapter of the CAFP\, serves as vice chair of CAFP’s Legislative Affairs Committee and is a faculty member of the CAFP’s Family Medicine Initiative on Trauma-Informed Care (FIT). She also represents CommuniCare+OLE on the Yolo County Health Council and in the Yolo County Trauma-Informed Network Of Care. Dr. Roshanravan enjoys spending time with her three elementary-aged children\, her husband\, and their dog. Her academic journey includes a bachelor’s degree in Behavioral Science from Drew University\, a Masters of Public Health in epidemiology from George Washington University\, and the prestigious attainment of her Project Management Professional accreditation in 2017. \nCloé Destinoble\, MPH\, Project Manager of Equitable Health and Well-Being\, National Association of Community Health Centers \nCloé Destinoble\, MPH serves as the Project Manager\, Equitable Health and Well-Being at the National Association of Community Health Centers. She currently helps health centers develop and implement data-driven strategies for documenting and addressing patient social risk factors through partnerships and focuses on building health center capacity to collect and act on standardized data on social drivers of health (SDOH). Cloé started her career in healthcare as she served in roles that aimed to optimize provider workflow and increase efficiency. Cloé then went on to serve as a health promoter by assisting with prevention of disease transmission by communicating audience-appropriate public health content\, both in writing (health messaging using technology) and through oral presentations\, while applying awareness of cultural values. Cloé later led projects that allowed her to develop and disseminate appropriate resources\, provide continuing education\, provide technical assistance and ran programs that support clinical care on the frontline of healthcare for underserved populations. She has also worked as an HIV peer educator for health departments and contributed as a Biological Scientist in their epidemiology departments. Cloé graduated from the Department of Health Promotion and Disease Prevention\, Stempel College at Florida International University with a master’s degree in Public Health with a concentration in Maternal and Child Health. SUMMARY:UDS+ Patient-Level Data Submission: Lessons Learned
DESCRIPTION:HRSA Uniform Data System (UDS) submission requirements\, processes and expectations are common knowledge across health centers and look-alikes. HRSA Health Center Program awardees and look-alikes are required to report a core set of information\, including data on patient characteristics\, services provided\, clinical processes and health outcomes\, patients’ use of services\, staffing\, costs\, and revenues as part of a standardized reporting system known as the Uniform Data System (UDS). Over the course of the past year and a half\, regular guidance and updated timelines have been shared with HIT vendors and health centers as they begin to prepare their systems for submitting annual UDS reports through new UDS+ FHIR based API pathway(s) that will now include patient level de-identified data. Raymond has been with CCALAC for over 11 years and is also CCALAC’s designated Project Director of its Health Center Controlled Network (HCCN) program. He has over 16 years of professional experience in the healthcare industry and is responsible for the oversight of CCALAC’s HCCN program and the effective coordination of CCALAC’s practice transformation technical assistance initiatives internally and with external partners. Raymond is the current chair of the California Primary Care Association’s HCCN Peer Network and is a contributing member of the national HCCN Peer Learning Team Advisory Committee\, which is supported by HRSA. Raymond holds a Master of Public Health from the University of California\, Los Angeles (UCLA)\, and a Bachelor of Science in Health Promotion and Disease Prevention Studies from the University of Southern California (USC). He is also a current Board Member of the UCLA Fielding School Health Policy and Management Alumni Association.  \nItohan Oyamendan\, Chief Operations Officer\, White Memorial Community Health Center \nMrs. Oyamendan is an experienced operations healthcare executive with a track record of leading successful practice initiatives\, driving process improvements\, maximizing productivity\, and introducing system efficiencies to propel growth. She has 20 years of health care and leadership experience within Federally Qualified Health Centers and Managed Behavioral Health Organizations serving the safety-net populations in Southern California counties\, most extensively in Los Angeles County. \nMrs. Oyamendan earned a Master of Health Administration from the University of Southern California and a Bachelor of Arts in Political Science from the University of California\, Riverside. \nStephen Gutierrez\, Chief Information Officer\, Northeast Valley Health Corporation \nStephen was hired by Northeast Valley Health Corporation in 2003 in the newly created position of Director of Information Technology and was promoted to Chief Information Officer in 2012. His main focus was in managing the growth and integration of technology across all sections of the corporation and developing a strategic guideline to follow for future expansion. The Information Technology department he leads consists of 15 staff and supports both hardware and software applications including network and telecommunications systems. \nIn the 21 years Stephen has been at NEVHC\, his department has been instrumental in implementing many electronic systems including Nextgen EPM/EHR\, Pre-visit Patient Engagement System\, Dentrix Dental EHR\, a QS/1 Pharmacy System with Interactive Voice Response system for 24hour prescription refills\, i2i Tracks Population Management System\, an automated reporting system\, automated outreach through text and voice communication and recently migrated the company to a hosted telephone system. Interoperability has been an on-going focus with Radiology vendors and Health Information Exchange networks. Stephen regularly attends the CCALAC HIT and Nextgen User Group meetings and is an active member of the CPCA forums. \nPrior to joining NEVHC\, Stephen worked for Fountain View\, Inc.\, a 50 site\, multi-state skilled nursing facility and Orthopaedic Hospital in downtown Los Angeles. His education includes a Bachelor’s degree in Business Administration with a concentration in Information Management from Cal State Fullerton. \nAnu Srinivasan\, Nextgen Applications Support Specialist & Reports Analyst\, Northeast Valley Health Corporation \nAnu joined Northeast Valley Health Corporation in 2019 and will reach her five-year mark in August. Anu is a clinician by training (OB/GYN)\, but due to circumstances not under her control\, quickly trained in Health Information Technology (HIT). She has gained immense insight & expertise using various EHR applications\, with Nextgen at the top of the list. \nAnu is the NG Support Specialist and Reports Analyst developing custom reports\, performing and coordinating NG application configuration changes in coordination with the CIO and HIT Support Manager. She is responsible for managing all NG custom template changes internal and external. The NG Support Specialist manages the configuration for patient outreach systems. Anu is also well versed with Rosetta interface connecting external vendors- Labs\, Mirth\, Nextgen Share etc. \nAnu’s strengths include her ability to learn and retain a tremendous volume of knowledge of the Nextgen software application. She has a strong desire to not only provide support for our care teams but to find the best possible solution to allow our organization to provide the best care we can provide. Anu is dedicated and always available to perform whatever job needs to be performed. She has blossomed in her ability to customize Nextgen templates and configurations. Anu is a key individual in the Information Technologies department to enhance the options of Nextgen. This last year she was instrumental in implementing the latest Nextgen upgrade and patches\, a custom Time Out process for Joint Commission\, automated Standing Orders and working with NG vendors on ECM\, Group templates and automation of PRAPARE and Pearls screening tools. \n

She specializes in portable development so that custom content is easily re-integrated during upgrades and truly enjoys teaching practice constituents on how to do their own smart development as much as doing the development herself. Her concentration is always on streamlining processes and “less clicks” for clinical providers and ancillary staff. \nJulie works with groups of all sizes from solo practitioners to large enterprises like MSOs\, ACOs\, FQHCs and national hospital organizations.
