DESCRIPTION:Conflict is unavoidable. Conflicts big and small can occur\, in our personal or professional lives and for multiple reasons. Whatever the reasons\, one thing is true and constant: knowing how to resolve conflicts before they escalate is crucial. SUMMARY:Recognizing and Addressing the Risks of Shifting Epidemiology of Adult Pneumococcal Disease
DESCRIPTION:Research shows that an increasing number of adults are experiencing pneumococcal pneumonia. In adults\, the same bacteria that causes the condition can also “invade” other areas of the body that are normally free from germs. When this happens\, it can affect the blood\, as well as the lining of the brain and spinal cord. This educational session provides an overview of a preventative vaccine available for both pediatric and adult populations. \nAs a result of participating in this session\, attendees will be able to: \n\nDiscuss the ongoing burden and shifting epidemiology of pneumococcal disease in adults\nReview the robust clinical trial program for an option to help address IPD in adults\nHighlight the 2024 CDC recommendations for adult pneumococcal disease\n\nPresented by Reena Gulati\, MD\, MPH\, Regional Medical Director\, Merck & Co.\, Inc \nComplimentary for CCALAC Members and Affiliates \nRegister Now! \nInstructor Biography: \nReena Gulati\, MD\, MPH\, is a Regional Medical Director employed by Merck & Co.\, Inc. In addition to her roles in vaccine development with the HIV Vaccine Trials Network and the Bill & Melinda Gates Foundation\, she spent nearly a decade with the Centers for Disease Control and Prevention focusing on the prevention and response to vaccine-preventable diseases. Dr. Gulati graduated from Stanford University\, earned her MD from Oregon Health & Science University\, completed her internal medicine training and chief residency at Stanford University Hospital\, and her infectious disease fellowship and MPH from the University of Washington. Linda also works as a private practice psychotherapist for individuals and couples and has taught workshops on LGBT Affirmative Couples and Managing Self Care as a Human Rights Advocate. In her free time\, Linda enjoys performing in cabarets\, swimming\, and crafting in the fiber arts. \nSara Watson\, MCHES\, CCALAC Training and Events Specialist \nSara Watson\, MCHES\, has worked at CCALAC as the Training and Events Specialist for the past four years and has over 15 years of experience in health education. Sara teaches mental health first aid and works with communities on CPR and first aid instruction. DESCRIPTION:Dr. Greg Vachon MD\, MPH\, Deena Pourshaban\, MPH\, and Leslie Brooks of Health Management Associates will co-facilitate a 60-minute webinar that will provide an overview of the fundamentals of empanelment\, describe the methodology of assigning patients to provider panels\, outline necessary staff roles to sustain empanelment processes and how to self-assess your own empanelment environment. Data analysis areas of focus include right-sizing and projecting workforce\, assessing impact of population interventions\, creating dynamic business plans in health care\, and supporting implementation and analysis of various components of optimized primary care. A component of special interest over the past two decades has been empanelment. Greg has assisted nearly fifty FQHCs and other safety-net systems across the country implement effective empanelment procedures reflective of their environments since 2010. \nDeena Pourshaban has over 12 years of healthcare and public health experience with extensive work in practice transformation\, social determinants of health\, quality improvement\, Patient-Centered Medical Home implementation\, health center operations\, and collaborative work with clients and care teams to tackle difficult clinical quality measures. Deena has led many large-scale practice transformation initiatives\, supported over 10 community health centers achieve PCMH recognition which includes implementing empanelment as one of the core requirements and recently co-lead the empanelment workstream supporting 32 community health centers implement participating under Kaiser Permanente Population Health Management Initiative implement empanelment processes and workflows. \nLeslie Brooks has over 8 years of experience collaborating with health plans and healthcare teams to implement whole-person\, person-centered approaches to healthcare integration. Her domains of expertise include population health management\, data analytics\, training and evaluation work. Ms. Brooks’ supported over 10 community health centers implement empanelment processes and workflows under Kaiser Permanente Population Health Management Initiative.
