Effective July 1, 2019, L.A. Care will launch the Medi-Cal Health Homes Program (HHP) for eligible beneficiaries with multiple chronic conditions who are frequent utilizers of medical services and may benefit from enhanced care management and coordination. HHP is a program designed by the Department of Health Care Services (DHCS) and authorized by the Centers for Medicare and Medicaid Services (CMS). The mandate to administer HHP is an opportunity for L.A. County to strengthen and grow point-of-care management capabilities. HHP will help leverage existing and facilitate new relationships with community-based providers to address and meet the needs of high-acuity Medi-Cal population.
This webinar, a collaboration between CCALAC, L.A. Care, and Health Net, will provide community clinics with an overview of HHP. Specifically, the webinar will provide information on the required HHP services, the Community Based Care Management Entities (CB-CME) network and the multidisciplinary teams that will be assigned to HHP enrollees, and provide community clinics the opportunity to discuss their potential role in HHP with L.A. Care and Health Net.
