The mission of T.H.E. (To Help Everyone) Health and Wellness Centers is to improve the health and well-being of the Los Angeles communities they serve by providing excellent and affordable preventive, primary and behavioral health care, accessible to all. A Federally Qualified Health center, T.H.E. has expanded its reach into the community to deliver care closer to where patients live by opening several new clinics in the last 18 months. Today, six T.H.E. Centers offer a full range of health care services to residents throughout South and Southeast Los Angeles. Earlier this year, T.H.E. was again recognized by the Joint Commission for its ongoing commitment to provide high-quality, low cost health care. After conducting a rigorous review of the organization’s main sites, the Joint Commission again issued its accreditation to T.H.E. of both its coveted Gold Seal of Approval ® and Primary Care Medical Home designation. These awards are only granted to health care entities that are in complete accord with the Commission’s rigorous health quality standards.
To bring care closer to where patients live, T.H.E. partnered with LAUSD to open two Wellness Clinics on-site at Dorsey High School and Crenshaw High School. These school-based centers serve students, their families and the general public to make access to preventive care easier. Most recently, T.H.E. celebrated the Grand Opening of its new La Brea Health Center in the Crenshaw/Baldwin Hills district of Los Angeles. Featuring state-of-the-art technology, including digital X-ray capability to service a new dental program there, this facility houses exam rooms, which will provide a space for primary adult and pediatric care. The health center also houses an education room for preventive care courses.
Since its founding in 1974, T.H.E. has been delivering culturally sensitive health care to its diverse, inter-generational community. Serving adults, seniors and pediatric patients, T.H.E. has a history of serving whole families for generations, offering wellness checks as well as a range of disease management programs to help patients manage and maintain their health. To extend outreach, T.H.E. brings care to the community with mobile clinics conducted throughout the month, providing free health screenings for hypertension, diabetes, cholesterol, obesity and HIV status. Certified enrollment counselors are on-hand to assist patients in enrolling patients in free or low-cost health plans. When health screenings indicate follow-up is required, T.H.E. staff proactively schedule appointments with care providers, connecting the mobile clinic to follow-up care at a center convenient for the patient. From pediatric to geriatric patients, T.H.E. provides quality health care to individuals across all stages of life, serving over 13,056 unduplicated patients and providing 45,746 encounters in 2014.
